Registration & Waiver

1. Assumption of Risk

I understand and acknowledge that ball hockey is a physical contact sport and that my participation involves certain inherent risks, including but not limited to: collisions with other players, contact with hockey sticks and balls, falls, and other impacts that may result in serious injury including permanent disability, paralysis, or death.

I freely accept and fully assume all such risks, dangers, and hazards and the possibility of personal injury, death, property damage, or loss resulting from my participation in ball hockey activities organized by Simcoe Ball Hockey League.

2. Release of Liability

In consideration of being permitted to participate in Simcoe Ball Hockey League activities, I hereby release, waive, discharge, and covenant not to sue Simcoe Ball Hockey League, its organizers, sponsors, supervisors, volunteers, participants, and persons transporting me to and from activities (collectively referred to as "the League"), from any and all liability for any loss, damage, or injury, including death, that I may suffer as a result of my participation in any activities.

3. Indemnification

I agree to indemnify and hold harmless the League from any loss, liability, damage, or costs, including court costs and attorney's fees, that the League may incur as a result of my participation in ball hockey activities, whether caused by negligence or otherwise.

4. Medical Authorization

I authorize the League to provide me with first aid and/or other medical assistance in the event of injury or illness. I understand that the League will attempt to contact my emergency contact, but if unable to do so, I authorize the League to secure medical treatment on my behalf.

I understand that I am responsible for all costs associated with medical care and transportation.

5. Photo and Video Consent

I grant permission to the League to use photographs, videos, or other media in which I appear for promotional purposes, including but not limited to websites, social media, and printed materials, without compensation.

6. Code of Conduct

I agree to abide by the League's code of conduct, which includes:

• Treating all players, officials, and spectators with respect
• Playing fairly and within the rules of the game
• Refraining from violent, abusive, or unsportsmanlike conduct
• Not participating under the influence of alcohol or drugs
• Wearing all required safety equipment during play

I understand that violation of the code of conduct may result in suspension or expulsion from the League without refund.

7. Equipment and Safety

I agree to wear all required protective equipment during games and practices, including but not limited to: helmet with face protection, gloves, shin guards, and athletic supporter (for males). I understand that failure to wear required equipment may result in being prohibited from play.

8. Acknowledgment

I have read this waiver and release form carefully and fully understand its contents. I am aware that by signing this form, I am waiving certain legal rights that I or my heirs, next of kin, executors, administrators, and assigns may have against the League.
